Instructions: To register your horse with CSI, please complete all of the following:

1. Submit this form
2. Submit a CSI Pedigree Form (or a photocopy of papers from another registry)
3. Email or mail 4 photos to clearly identify markings (one front, one back, one from each side)
4. Submit your registration fee via
   
   or mail. Fee for registration horse under 1 year old is $25.00 founding member, $35.00 member, $50.00 non member.
   Fee for registration horse over 1 year old is $35.00 founding member, $45.00 member, $75.00 non member.
